Summary: The clinical pharmacist provides services for centers and clinics operating from within the college. This position provides in-person, virtual, and phone-based clinical consultations with patients referred to the centre or clinic. This includes the provision of medication assessments for complex general medicine patients and for patients with non-medication therapy for insomnia and pharmaceutical care for patients who have chronic non-cancer pain. Additionally, the pharmacist supervises pharmacy students in both clinical settings. This role works as part of a team of healthcare professionals that provide a variety of chronic pain focused on direct patient care clinical services, both in person and remotely, for Saskatchewan residents. Duties and Responsibilities: Provides direct patient care including pharmaceutical care and comprehensive medication assessment to patients who are referred to the clinic and centre. Conducts comprehensive medication reviews for patients with complex medical conditions, supporting safe and effective therapy. Collaborates with other health professionals in the care of individual patients referred to the clinic and centre. Provides patient education on medication use, side effects, and adherence strategies. Monitors and evaluates therapeutic outcomes, adjusting treatment plans as needed. Participates in interdisciplinary case conferences and contributes to clinical decision-making. Documents clinical interventions and recommendations in patient records according to regulatory and institutional standards. Supports research and quality improvement initiatives, including data collection and analysis related to medication therapy. Develops and delivers educational sessions for healthcare teams, students, and patients on pharmacotherapy topics. Complies with professional standards, legislation, and institutional policies related to medication management. Provides input and feedback to policy and guideline development for medication use within the centre or clinic. Supervises undergraduate students within the pharmacy academic program in the provision of direct care, providing guidance, mentorship, feedback, and participating in assessment. Maintains current knowledge of evidence-based practices in pharmacotherapy and chronic pain management. Undertakes related duties, as assigned, consistent with the nature of the position. Qualifications: A bachelor’s degree in pharmacy or Doctor of Pharmacy (PharmD). Licensed or eligible to be licensed to practice pharmacy in Saskatchewan. Additional training or credentials are an asset. Minimum of three years of pharmacy practice experience. Experience providing comprehensive medication assessments with complex patients, insomnia management, and or managing complex patients with chronic non-cancer pain is preferred. Experience supervising students in a clinical setting is an asset. Strong professional pharmacist skills, including patient interviewing, empathetic communication, identification of drug therapy problems, creating care plans, and documentation.
