Care Coordination Supervisor

The Housing Works Health Home Care Coordination Program partners with medical and behavioral health providers to deliver coordinated, high-quality care for Medicaid recipients with complex health needs or those at risk of high service utilization. Rooted in improving access to care, reducing barriers, and supporting long-term health outcomes, we provide client-centered, strengths-based services that meet individuals where they are.
