What We’re Looking For
Trillium Health Resources has a career opening for an RCT (RB-BHT) Care Coordinator to join our team! The RCT (RB-BHT) Care Coordinator is responsible for providing short-term care coordination and service linkage for individuals diagnosed with autism spectrum disorder who are either not eligible for or have opted-out of Tailored Care Management and have co-occurring chronic, unresolved or complex physical, developmental disabilities, behavioral health and social determinant needs.
The RCT (RB-BHT) Care Coordinator will conduct outreach to members and their legal guardians to assess unmet needs and connect them with the most appropriate services and resources. The Care Coordinator is expected to complete all required documentation, paperwork, and tasks within Trillium’s care management platform according to established timelines.
This position will serve as a member of the integrated care team and provide input and collaboration with other team members as needed. The role is essential in assisting members and/or legal guardians in obtaining the needed services and supports, identifying available housing, transportation, nutritional resources, and communicating with providers to promote coordinated and integrated whole person care.
On a typical day, you might:
- Serve as a resource for accessing community-based resources and supports.
- Assist with coordinating services and support for members.
- Engage identified members into Care Coordination and utilize data and information from multiple sources to address members’ needs.
- Provide care coordination to assigned members with identified behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions.
- Provide education about all available services, natural and community support, treatment options, diagnosis, etc.
- Facilitate referrals for health-related services, community-based resources and social support services.

Qualifications
Required:
- High School diploma/GED and six (6) years of experience working with BH/MH/SU/IDD population; OR
- Associate’s degree and four (4) years of experience working with BH/MH/SU/IDD population; OR
- Bachelor’s degree and two (2) years of experience working with BH/MH/SU/IDD population; OR
- Equivalent combination of education and experience.
- Must have a valid driver’s license and reliable transportation.
- Must reside within Trillium’s catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, or Wilson.
- Must be able to travel within catchment as required.
