Bilingual Lead Care Manager

Enhanced Care Management (ECM) is a Medi-Cal managed care benefit that supports the coordination of services, establishes connections to community resources, and provides comprehensive care management. The Lead Care Manager serves as the primary contact for youth and families who need support with care coordination and community linkages. This position works as part of a broader ECM team across different counties, engaging regularly with managed care plan partners and youth and families to support access to services.
