How you’ll make an impact in this role

• Coach, evaluate, and develop your case management team, driving a high-performing culture of advocacy and clinical excellence.
• Oversee daily workflows, streamline case management processes, and champion efficient resource utilization and discharge planning.
• Serve as the primary escalation point for complex, high-risk patient care tracking, offering expert guidance on challenging case barriers.
• Act as a preceptor for new case managers, tailoring orientation plans to set them up for success in navigating compliance and care transitions.
• Collaborate closely with leadership on daily operations, contributing valuable insight to hiring, training, and departmental performance.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Required Credential(s):
• Registered Nurse obtained prior to hire date or job transfer date.
• Preferred Credential(s):
• Case Manager credentialed from the American Case Management Association (ACMA).
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required. Master's degree preferred.
