The Children and Family Health Institute is a charitable, nonprofit program of VNA Health Group focused on improving the health and well-being of children and families through accessible, community-based care. CFHI delivers high-impact programs including nutrition support, preventive screenings, vaccinations, and inclusive health services through community health centers and mobile outreach. Position Summary:
The Case Manager plays a key role in expanding access to mental and behavioral health support for uninsured and underinsured CFHI clients. As part of the MindCare Now initiative, you’ll help clients connect with licensed mental health providers through the Talkspace telehealth platform, ensuring they receive timely, culturally responsive, and compassionate care. You’ll work closely with the Community Health Worker to guide clients across CFHI programs, build trusting relationships, reduce barriers to care, and support ongoing engagement in virtual behavioral health services. Your work strengthens access and equity by addressing social needs, supporting digital access, and providing culturally responsive outreach.
Principal Responsibilities:
Contribute to CFHI’s mission by fostering a positive, collaborative, and supportive work environment.
Conduct assessments to identify client needs and develop individualized care plans for telehealth-based behavioral health services.
Work proactively with clients to identify and overcome real or anticipated barriers to care.
Provide referrals to additional services as needed, including housing support, substance use services, and other community resources or entitlements.
Maintain accurate and timely documentation in the designated reporting system; prepare reports as requested by the Project Director.
Provide ongoing follow-up and evaluate the effectiveness of services to ensure clients receive the support they need.
Follow all agency infection control and safety policies.
Adhere to organizational policies regarding attendance, professionalism, and appearance.
Job Competencies & Minimum Qualifications:
Bilingual Spanish strongly preferred
Bachelor’s degree required
1–2 years of experience in case management, public health, or a related field
Access to a car and a valid driver’s license

Working Conditions/Physical Demand:
Works indoors/outdoors with client/customer contact and with use of business office machines such as: telephone, computer, printer, copier, fax machine, email, and voicemail
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $30,000.00 to USD $65,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
