PATIENT NAVIGATOR - PRIMARY CARE | AMBULATORY CARE CENTER | FULL-TIME DAY SHIFT (25428)

The goal of the Patient Navigator (PN) is to navigate newly diagnosed patients at BNBMC and surrounding communities to receive timely, patient-centered care at BNBMC. The PN will engage patients, creating a trusting relationship, navigate patients’ appointments and closely communicate with a patient’s entire care team based on clearly outlined goals and identified patient needs.
