Job Description
We are looking for a compassionate and driven Case Manager to join our Day Program team, where you'll make a meaningful impact on the lives of individuals with intellectual and developmental disabilities. In this role, you’ll guide and support a dedicated team, ensuring a safe, engaging, and person-centered environment. You’ll oversee client care, facilitate enriching training opportunities, and serve as a key advocate—building strong connections with families, caregivers, and community partners to promote success and independence.
Location: El Cajon
Schedule: Mon- Fri 8a-4:30p
Pay: The pay range for this position is $24.00 to $24.00
Who You Are:
A dedicated advocate passionate about supporting individuals with developmental disabilities
A collaborative leader who mentors and supervises Direct Support Professionals (DSPs)
Skilled in person-centered support, ensuring clients receive quality care and resources
Organized and detail-oriented, maintaining accurate client records and compliance with regulations
A Typical Day Will Include:
Managing a caseload of clients, ensuring individualized care and support
Developing and implementing Individual Support Plans (ISPs) that foster independence and inclusion
Supervising and training DSPs, providing mentorship and performance evaluations
Coordinating enriching program schedules and facilitating goal-setting meetings
Ensuring compliance with Community Care Licensing, Title 22, and Title 17 regulations
Advocating for clients by networking with care providers, families, and funding agencies
What You’ll Need to Be Successful:
High School Diploma required;
Bachelor's degree in Social Services or related field preferred
Supervisory experience
Experience in case management, record-keeping, and client evaluations
Knowledge of SDRC referral and funding procedures
Familiarity with Community Care Licensing and California Code of Regulations, Titles 17 & 22
Valid California Driver’s license and ability to transport individuals as needed

Have experience using electronic case management systems (Therap preferred)
Are Bilingual (English-Spanish)
Are passionate about team leadership, advocacy, and quality assurance
