Job Description
Position Summary
The Advanced Practice Provider (APP) for the Center of Comprehensive Heart Failure Care plans and implements all clinical and business efforts to promote heart health, assure excellent clinical care and reduce HF all cause 30-day readmissions. The APP facilitates patient transitions within and between health care settings. The APP will also coordinate education and outreach, working in collaboration with Home Care, Nursing Homes, Physicians, and other multidisciplinary teams to assure successful outcomes. The APP will perform inpatient and outpatient consultations, based upon established Departmental Protocols and ensure patient access to all appropriate resources. The APP will promote patient independence by establishing patient care goals, teaching and counseling and reinforcing understanding of disease processes, medications, and self- care skills. The APP will participate in activities that contribute to the education of other healthcare professionals including precepting, presenting and publishing.
Education
Graduate of an accredited Physician Assistant program as determined by the Committee on Allied Health. Education and Accreditation or a current Registered Professional Nurse and Graduate of an accredited Nurse Practitioner Program/master’s in nursing. Current Certification from the National Commission on the Certification of Physician Assistants (NCCPA) or for Advanced Practice Nurses a current Board Certification from the American Nurses Credentialing Center (ANCC) or from the American Academy of Nurse Practitioners (AANP)
Experience
Minimum of 1-3 years of clinical experience, primarily in area of specialty.
Skills
Hold a valid, current New Jersey license. Obtain a New Jersey CDS and federal DEA certificate upon employment if not already covered. Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving. Participation in pertinent professional and/or specialty organizations preferred. BLS ACLS certification. Serve on call and / or weekend rotation as required.
Salary
Minimum new grad base salary $143,200 K. Salary commensurate with years of experience as an NP/PA.
