Care Manager

The Care Manager will provide all aspects of care coordination for SAMHSA Health Pathways clients: complete initial enrollment (all required SAMHSA and program forms/assessments, including the SURPT Tool), develop care plans, promote compliance with appointments and treatment regimens, and encourage self-sufficiency.
