Care Coordinator – IHPA

The Care Coordinator provides comprehensive care coordination to attributed Illinois Health Practice Alliance (IHPA) members assigned to Metropolitan Family Services. The services that are provided enhance the access of members with complex behavioral needs to integrated behavioral, medical and social services.
