Wound Nurse RN or LPN

Castleton Health Care Center is seeking a compassionate and skilled Wound Care / Treatment Nurse (RN or LPN) to join our dedicated team! This is a great opportunity for a nurse who is passionate about wound care, enjoys working in a collaborative environment, and wants to make a difference in the lives of our residents.

This position includes participation in a weekend on-call rotation.

What You’ll Do:

• Provide Essential Treatments: Administer physician-ordered treatments, including wound care, IV therapy, and specialized clinical interventions.

• Monitor Patient Progress: Conduct regular assessments of resident conditions, documenting vital signs, healing progress, and any notable changes.

• Collaborate with Care Teams: Work closely with physicians, fellow nurses, and interdisciplinary team members to develop and implement personalized treatment plans.

• Educate and Support: Help residents and their families understand care plans, medications, and follow-up routines.

• Maintain Accurate Records: Ensure timely and complete documentation in compliance with state regulations and facility policies.

• Promote Safety & Best Practices: Adhere to infection control guidelines and clinical safety protocols.

Qualifications:

• Current RN or LPN license in the State of Indiana.

• CPR/BLS certification required.

• Experience in wound care, IV therapy, and treatment planning is strongly preferred.

• Prior work in a long-term care or skilled nursing facility is a plus.

• Excellent communication, organization, and teamwork skills.

• A compassionate, patient-focused approach and attention to detail.
