At the present time, a vacancy exists for a Case Specialist within the Student Services division of the Chester County Intermediate Unit. The Case Specialist is part of the Home and Community Services Department and will be responsible for overseeing all clerical related details on their caseloads. Due to the nature and location of the services provided, the incumbent will have a mid-level of "freedom to act" which will require a person who is self-energized, self-reliant, and able to make decisions within the arena.

Summary of Position

This is position, working with Home and Community Services Intensive Behavior Health Services program, is responsible for coordinating services to clients of Home and Community Services (previously referred to as "Wraparound services") for pre-school and school aged children diagnosed with emotional/behavioral disorders. The incumbent will assist eligible individuals in gaining access to necessary medical, educational, rehabilitative, and social services; assist in monitoring service delivery. The Home & Community Case Specialist will be responsible for: all clerical duties related to the position, ensuring the timely development and submission of client-centered plans; promoting positive relations with referral, regulatory and funding agencies; and promoting the integration of children with disabilities in the community. The Home & Community Case Specialist will participate in the client intake process and the orientation of clinical staff, including Behavior Health Specialist, Behavior Consultants, Behavior Consultants-ABA, Behavior Analysts, Mobile Therapists and Supervisory Case Managers, and other Case Specialists.

ESSENTIAL JOB RESPONSIBILITIES: 

  1. The incumbent will perform day-to-day case maintenance, which includes all clerical responsibilities related to the promptness of packet submission and the receipt of service authorization from all identified funders/insurances, etc.
  2. The incumbent will ensure the timely development and implementation of individual treatment plans as well as monitoring packets to ensure compliance with, but not limited to State, Federal, and local regulations.
  3. Will conduct service intakes for clients.
  4. Will work with a consultant pool to ensure appropriate staffing of caseloads.
  5. Will meet with the Interagency Team, including the child/family, to develop a summary of care.
  6. Will coordinate and oversee delivery of services delineated in plan of care summary whereby individualized plans can be carried out.
  7. Will monitor the billing system to ensure reimbursement from Medicaid ((both through the Behavioral Health Managed Care Organizations (BHMCO) and fee-for-service)) and private insurers for services provided to clients.
  8. The incumbent will maintain a strong professional relationship with the professional community.
  9. Coordinator in maintaining contact and dialogue with early intervention, school district, and other community organization personnel to assure that there is a working relationship, which will enable the child/adolescent to function optimally in these settings.
  10. To assist in the clerical duties associated with H&CS Training to the amount and extent that is needed programmatically.

QUALIFICATIONS:

  • High school diploma or G.E.D required
  • Some experience with Medical Assistance (MA) required.
  • Experience working in case management preferred.
  • Experience working in the behavioral health field preferred.
  • Must have knowledge and skill in developing trusting relationships and communicating clearly and concisely with children, adolescents, families, subordinates, and peers toward problem solving, planning, and resource access.
  • Must be skilled in supporting staff to assist clients in taking an increasingly self-determining role in home, school, and community contexts as they transition to community independence.
  • High problem-solving skills within the scope of customer service and clinical service.

 

Hourly Rate (2026-2027 SY):
Project Staff, Level 11, Nonexempt $25.14/hr. to $31.49/hr.
*rate determined by years of experience in a similar role
SCHEDULE: Monday - Friday; 7.5 hrs/day; 260 days/year
