Occupational Therapist (OT) – Home Health
Agency: Baptist Health Home Care Kentucky
Work Schedule: Monday–Friday (5 days/week)
Territory: Lexington, Salvisa, Lawrenceburg, Sadieville, Frankfort, KY
Sign-On Bonus: $5,000

Position Summary
The Occupational Therapist provides skilled therapy services in patients’ homes under physician orders. This role is responsible for evaluating patient needs, developing individualized treatment plans, implementing therapeutic interventions, and ensuring safe, effective, and compliant care delivery.
The OT also educates patients and caregivers, supervises Certified Occupational Therapy Assistants (COTAs), and ensures all documentation meets agency and regulatory requirements.
Key Responsibilities
Clinical Care & Treatment Planning
- Provide skilled occupational therapy services according to the physician’s plan of care.
- Develop and adjust individualized treatment plans based on patient progress and changing needs.
- Observe, document, and report patient responses, changes, and outcomes to the physician and care team.
- Evaluate patients using appropriate diagnostic and prognostic methods.
Patient Outcomes & Rehabilitation
- Use therapeutic activities and home exercise programs to improve strength, coordination, range of motion, and functional independence.
- Support recovery through progressive exercise and functional activity training.
- Help reduce hospital readmissions through patient education and preventive care strategies.
Patient & Family Education
- Educate patients and families on diagnosis, treatment plans, and self-management strategies.
- Instruct on the safe and proper use of adaptive equipment such as walkers, wheelchairs, canes, braces, crutches, and prosthetic or orthotic devices.
- Provide counseling and guidance throughout the rehabilitation process.
Supervision & Collaboration
- Supervise and support COTA-provided patient care.
- Communicate regularly with COTAs regarding supervisory visits and patient progress.
- Collaborate with physicians, therapists, nurses, and other healthcare professionals to ensure coordinated care.
Documentation & Compliance
- Complete accurate and timely clinical documentation in accordance with agency policies and Medicare/Federal guidelines.
- Maintain compliance with all applicable regulations, procedures, and scope of practice standards.
- Communicate scheduling or availability changes with appropriate staff.
Professional Responsibilities
- Participate in in-service training and mandatory agency meetings.
- Maintain active licensure and professional competency.
- Perform additional duties as assigned.
Qualifications
Required
- Current Occupational Therapist (OT) license in the state of employment.
- Minimum one (1) year of OT experience in an acute care setting.
- Valid driver’s license and current automobile insurance (listed as a driver).
- Ability to meet the physical demands of home health care.
Preferred
- Previous home health experience.
