Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans; clients connected to the criminal justice system and outpatient care to anyone needing assistance with addiction and/or mental health treatment.
Position:
Our Crisis Case Manager plays a hands-on role in providing quality care and support to individuals in short-term residential mental health crisis stabilization.
Schedule: Monday–Friday, 8:00 AM–4:00 PM (flexibility as needed)
On-Call: Participation in an on-call rotation is required

Key Responsibilities:
- Provide individualized care that protects consumer rights, dignity, and wellness
- Support individualized crisis stabilization response and service planning
- Facilitate daily individual and group sessions (non-therapeutic)
- Document behaviors, interventions, and progress within 24hours
- Coordinate care and communicate updates with internal crisis team
- Participate in staffings and professional development sessions
- Maintain compassionate, professional relationships with clients and team
- Maintain a minimum billing rate of 66% through timely and accurate documentation
Qualifications:
- Bachelor’s degree in mental health, human services, or related field preferred
- CNA certification may be considered with relevant experience
- Experience working with adults in crisis or with co-occurring disorders
- Familiarity with trauma-informed care and recovery-based practices
- Strong judgment, problem-solving, and communication skills
- Working knowledge of computers, or willingness to learn
- Valid driver’s license and clean driving record
- Must complete TB test, and drug screening (on-site).
- Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days
- Employment is contingent upon passing a state and federal background check.
