CMHA is looking to hire full-time Rehabilitation Counselor. This position will be located at 26 Russell St Britain, CT 06051. Monday-Friday, 8:30am-4:30pm. 

Compensation Range: The salary range for this position starts at $20.00.

Compensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. CMHA also takes into consideration our benefits and paid time-off package to provide a well-rounded and competitive approach to the applicant’s overall compensation.

Summary:

Takes primary responsibility to collaborate with membership with prolonged mental illness and/or dually diagnosed to develop rehabilitative services to assist them to live, learn, socialize, work and be independent in the community. Collaborates with Psychosocial Rehabilitation Team on daily groups, activities, and skill building with program participants.

Essential Responsibilities and Target Outcomes:

  • Coaches and assists clients with self-advocacy by monitoring and supporting client contact with outside entities (DMHAS, DSS, landlords, Employers, social networks, Other Community Support Providers)
  • Completes intakes, discharges, and direct services (case management) with clients to achieve an independent and self-sustaining social network.
  • Performs outreach to membership to monitor progress and encourage social connection.
  • Supports psycho educational and skill building groups with membership.
  • Collaborates with integrated clinical teams
  • Provides program transportation as needed, and serves as support for Transportation department.

Other duties as assigned.

Education:

  • High School Diploma or equivalent or higher.

 

Credentials and Licenses:

  • Must have valid driver’s license and be able to obtain a public service license. 
  • Certified in First Aid and CPR. MANDT training upon hire.

Experience:

  • At least 1 year of experience working with severe and prolonged mentally ill/or dually diagnosed individuals.
  • Demonstrates beginner to intermediate experience with Microsoft Office products including Word and Outlook.  Experience in or the ability or willingness to learn how to use an Electronic Medical Record (EMR) system, which includes appointment scheduling, authorization tracking, assessments, treatment plans, progress notes, and discharge summaries

  • Demonstrated awareness/sensitivity to all types of issues related to client diversity, including cultural, ethnic, language, gender, sexual orientation, gender preference, physical handicap and spiritual diversity.
