TBI Waiver Marketing & Coordination Specialist

White Glove Community Care an established homecare agency is seeking a knowledgeable, motivated, experienced and dedicated professional with in-depth knowledge of New York State’s Traumatic Brain Injury (TBI) Waiver Program to join our Brooklyn team. The TBI Waiver Coordination Specialist will play a key role in facilitating service coordination, managing referrals, and ensuring participants receive the appropriate supports through the waiver.

While this role includes outreach and networking responsibilities, priority will be given to candidates with direct TBI Service Coordination (SC) experience and a solid understanding of waiver procedures, documentation, and compliance standards.

Key Responsibilities:

• Coordinate and oversee TBI Waiver referrals, intakes, and participant transitions.
• Serve as a primary point of contact between participants, service coordinators, providers, and state agencies.
• Provide guidance and support to ensure participants’ needs are effectively identified and addressed in accordance with waiver requirements.
• Maintain accurate documentation, track service coordination activities, and prepare required reports.
• Ensure compliance with all TBI Waiver policies, procedures, and confidentiality regulations.
• Build and sustain strong collaborative relationships with hospitals, rehabilitation centers, and community providers.
• Participate in limited outreach and community engagement activities to strengthen program visibility and referral sources.

Qualifications:

• Required: Proven experience working within the New York State TBI Waiver Program.
• Comprehensive knowledge of waiver eligibility, service coordination protocols, and provider collaboration.
• Strong ability to manage participant caseloads and coordinate complex service needs.
• Exceptional communication, organization, and recordkeeping skills.
• Ability to work independently and collaboratively in a team-based, community-focused environment.

Work Schedule:
Full-time, on-site position based in our Brooklyn office.
