Bay Pointe Nursing Home is seeking a compassionate, organized, and dedicated Social Work Assistant to join our team. This role supports residents and their families by helping coordinate care, providing emotional support, and ensuring a positive living environment.
Key Responsibilities:
Assist the Social Services Director with resident assessments and care planning
Provide support to residents and families during admission, stay, and discharge
Help coordinate community resources and services
Maintain accurate and confidential documentation
Support resident advocacy and ensure psychosocial needs are met
Participate in interdisciplinary team meetings
Qualifications:
Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred
Previous experience in long-term care, healthcare, or social services is a plus
Strong communication and interpersonal skills
Compassionate, patient-centered approach
Ability to handle sensitive situations with professionalism
