Medical Case Manager

The Medical Case Manager (MCM) will provide all aspects of care coordination for the AIDS Institute (AI) Advancing Access Case Management clients: complete initial enrollment (all required AI and program forms/assessments), develop care plans, promote compliance with appointments and treatment regimens, and encourage self-sufficiency.
