Social Worker

Reporting to the Program Manager, the Social Worker will be responsible for, but not limited to, leading the case management services provided, supervising two case managers, providing training and clinical support to the CRED care team, conducting psychosocial assessments with newly enrolled clients, providing individual and group level counseling, ensuring all clients have co-developed, comprehensive service plans and overall ensuring CRED Program clients are connected with mental health support services needed.
