This role includes evaluating patient needs, implementing treatment plans, and ensuring safe, high‑quality care in the home environment.What You’ll DoProvide occupational therapy treatments as ordered by the physicianAssist in developing and updating the Plan of Care (POC)Evaluate patient functional levels and progressObserve, record, and report patient condition changesTeach and guide family members and Lorian staff on therapy programsSupervise other healthcare personnel when appropriateAssess the home environment and recommend modificationsParticipate in in‑service education programsComplete and submit accurate, timely clinical documentation per Lorian policiesNote: Documentation must be accurate and complete for visits to be payable.What You Need (Qualifications)Graduate of an accredited occupational therapy programRegistered with the Occupational Therapist Association; licensed in state of practice (as applicable)Minimum 2 years of relevant OT experiencePre‑employment health clearance, including TB test/CXR (per state requirements)Able to read, follow written instructions, and document care accuratelySelf‑directed with strong independent work skillsCurrent CPR certificationProfessional, flexible, and demonstrates tact and patienceGood physical stamina and mental healthValid driver’s license and insured personal vehicle (per Lorian Health requirements)Able to document all client‑related activityWhy Join us?We are committed to our employees' well-being, offering a comprehensive employee benefits package that includes, but is not limited to:Competitive salary and comprehensive benefits package (medical, dental, vision, life, AD&D, disability, EAP, 401(K) with 4% matchPaid time off and paid holidaysMileage reimbursement or company vehicle (role dependent), plus fuel cardEmployee referral program and opportunities for growth and advancementThis position is based on a Pay per visit compensation model.Pay per visit ranges from $90 to $120 visit commensurate with experience and depending on type of visit.
