Position Summary
Provide medical services in Grace Clinic and clinical support Center-wide to assure quality patient care under the supervision and collaboration of a licensed physician. Collaborate with physicians, nurse practitioners, nursing staff, and interdisciplinary treatment teams regarding general medical issues, patient histories, physical findings, and treatment planning.

• Obtain relevant physical health history for assigned patients
• Evaluation and treatment of general/medical illnesses under the collaboration of the physician
• Performs other duties as assigned by the Chief Medical Officer
• Assume primary on-call responsibilities. This duty includes the provision of outpatient primary care services./FQH
• Will take care of walk-in cases and tele-med care as needed
• Flexibility in work schedule as determined by organizational needs
• Provision of data and quality metrics as required by FQHC Program of HCI
Minimum Qualifications/Requirements
• Graduation from an accredited and approved Physician Assistant program
• Possess Certification by the National Commission on Certification of Physician Assistants (NCCPA)
• Successfully meets criteria for credentialing and privileging if applicable.
• Active collaboration with referrals to community physicians for quality care for HCI consumers
• Time management skills.
• Effective written and oral communication skills.
• Effective interpersonal skills.
• Willingness to use personal transportation in work capacity.
Certificates, Licenses, Registrations
• Possess a current State of Indiana Physician Assistant license
• Possesses a current CSR and DEA License to prescribe controlled substances in Indiana
• Maintain current American Red Cross CPR/1st Aid certification
• Maintain current Crisis Prevention Intervention (CPI) certification
• Valid Driver’s License in accordance with motor vehicle policy
• Licensed and board certified or board eligible as defined by federal and Indiana state law
• Completes all required continuing education requirements in a timely manner
Phyical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Exposure to possible agitated patients is limited but possible. Job stress ranges from low to intense depending on situation.
• Standing, walking, bending, stooping as required to provide individualized patient care.
• Possesses strength necessary to direct and assist in implementation of CPI interventions.
• Able to drive mobile unit to community partners and agencies per organizational and FQHC needs.
Work Enviornment
Most of the work of the Physician Assistant is done directly with consumers in Center facilities, which provide a comfortable physical environment with consumers. On occasion the Physician assistant may see clients either in their homes or in community settings. Office and program areas may not be air-conditioned and could become overly warm in summer months. Work conditions, such as cleanliness, comfort and safety, may vary widely. Evening and/or weekend work may be required. Smoking is permitted only outside of program and office sites. Safety management and crisis intervention decisions and actions are dependent on staff present. Crises may arise, which may be stress producing and/or be exhausting.
