Moraga Post Acute is seeking an experienced and detail-oriented MDS Coordinator RN to join our clinical leadership team. Position Summary
The MDS Coordinator RN is responsible for coordinating and completing resident assessments in accordance with Medicare, Medicaid, OBRA, and facility requirements. This position ensures accurate and timely completion of the MDS process, care planning, documentation, and reimbursement coordination while supporting nursing operations and regulatory compliance.
Essential Duties & Responsibilities
MDS & Care Coordination
Coordinate and oversee the completion of resident assessments and MDS schedules
Ensure assessments are completed accurately and within required timelines
Develop, coordinate, and update individualized resident care plans
Manage Medicare and Medicaid assessment scheduling and coverage processes
Assist with reimbursement documentation and denial notifications as needed
Maintain current knowledge of Medicare and Medicaid regulations and updates
Clinical & Administrative Responsibilities
Support day-to-day nursing operations in accordance with facility policies and regulatory guidelines
Collaborate with interdisciplinary departments to ensure continuity of resident care
Participate in quality assurance and performance improvement initiatives
Assist with discharge planning and resident transitions
Participate in state and federal surveys and audits as requested
Complete administrative reports, medical forms, charting, and evaluations as required
Maintain confidentiality of resident and facility information in compliance with HIPAA regulations
Documentation & Charting
Ensure accurate and timely clinical documentation for admissions, transfers, and discharges
Review physician orders, treatment plans, and resident records for accuracy and compliance
Maintain detailed nursing notes and incident reports
Ensure all charting meets facility, state, and federal standards
Medication & Clinical Oversight
Administer medications and treatments as ordered, when necessary
Monitor medication records and narcotic documentation for accuracy
Report discrepancies and ensure compliance with medication administration policies
Team Leadership & Staff Support
Assist with staffing coordination and shift planning
Support nursing staff compliance with policies, procedures, and job responsibilities
Participate in employee evaluations and staff development
Communicate staffing concerns and operational needs to nursing leadership
Qualifications
Current and active California Registered Nurse (RN) license
Previous MDS Coordinator experience preferred
Strong understanding of Medicare, Medicaid, OBRA, and skilled nursing regulations
Experience in long-term care or post-acute care preferred
Excellent organizational, communication, and leadership skills
Ability to manage multiple priorities in a fast-paced environment
Knowledge of electronic medical records and clinical documentation systems
