Pay: $80 per visit

Location: O’ahu, HI (reliable transportation required)
Schedule: Flexible / Per diem — choose your patients

Always Best Care is a Medicare-certified Home Health agency dedicated to helping patients remain safe, independent, and active in their own homes. Our therapy team plays a vital role in supporting seniors as they improve mobility, strength, and overall quality of life. 

We are currently seeking licensed Physical Therapists to provide home health services across O‘ahu.

Key Responsibilities:

• Conduct skilled therapy visits in patients' homes
• Perform PT evaluations and follow-up assessments
• Develop fall prevention and safety strategies
• Implement mobility, balance, and strengthening programs
• Educate patients and family caregivers
• Complete accurate Medicare-compliant documentation

 

Qualifications:

• Current Hawaii license as PT 
• Previous home health experience is a plus
• 1+ year clinical experience (home health preferred)
• Strong documentation and communication skills
• Driver’s license + reliable transportation
• Excellent clinical, communication, and organizational skills
