Join a team that prioritizes growth, open communication, and the tools you need to make a meaningful impact in the lives of others.Location: Sacramento Preferred Availability: 10–20 hours per week, including rotating holidays, on-call shifts, and weekends.On-Call Hours: 6:00 PM – 9:00 AMAbout the Role We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.ResponsibilitiesProvide nursing care according to the RN‑developed Plan of Care.Monitor and document changes in patient condition; report to RN/physician as needed.Assist with activities of daily living to promote patient independence.Educate patients and caregivers within LPN/LVN scope of practice.Maintain accurate EMR documentation.Participate in case conferences and coordinate care with the home health team.Follow infection‑control, HIPAA, and company policies.Required QualificationsCurrent LPN/LVN license in the state of practice.Graduate of an accredited practical/vocational nursing program.Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Current CPR/BLS certification.Valid driver’s license, reliable insured vehicle.Strong communication, clinical judgment, and ability to work independently.Why Join us? We are committed to our employees' well-being, offering a comprehensive employee benefits package that includes, but is not limited to:Competitive salary and comprehensive benefits package: EAP and 401(K) with 4% match, accrued sick hours, mileage reimbursementEmployee referral program and opportunities for growth and advancementThis position is based on an hourly visit compensation model.Hourly visit ranges from $33 to $42 visit commensurate with experience and depending on type of visit
