Victorian Post Acute is a beautiful 90-bed facility located in a prime area with ample parking. We are a rehab-focused team serving both short-term and long-term patients. We are currently seeking a passionate and motivated Occupational Therapist to join our dynamic in-house therapy team.

Starting Rate: $53 per hour (dependent on experience)

Key Responsibilities:
Develop and implement effective treatment plans in coordination with referring physicians.
Provide direct therapy services according to the approved plan of care.
Train and support Restorative Aides within the nursing department.
Supervise Occupational Therapy Assistants (OTAs) in patient care and documentation in line with state practice guidelines.
Collaborate with supervisors and interdisciplinary team members to monitor patient progress and adjust treatment as necessary.
Participate in care conferences, utilization reviews, and rehabilitation meetings as required.
Maintain accurate and timely documentation, including evaluations, daily notes, progress reports, and recertifications according to regulatory standards.
Educate patients, families, and caregivers on home programs and discharge planning to promote continued independence.
Recommend and coordinate the use of durable medical equipment (DME) to support safe mobility and function (e.g., braces, walkers, grab bars).
Promptly report equipment issues to ensure a safe and functional work environment.
Qualifications:
Licensure: Valid Occupational Therapist License (State of California)
