As an RN Clinical Coordinator at LifeCircles PACE, you’ll provide patient-centered nursing care across clinic, home, and facility settings while enjoying a schedule that promotes work-life balance. Partnering with an Interdisciplinary Team, you’ll provide holistic, high-quality care that improves outcomes and helps older adults live with dignity, independence, and confidence.
Key Responsibilities
- Deliver Direct Nursing Care – Assess acute needs, report changes in condition to the clinical team, and complete skilled nursing tasks including wound care.
- Conduct Health Assessments and Adjust Care Plans – Complete periodic health assessments and participate in interdisciplinary care planning.
- Coordinate Interdisciplinary Care – Participate in daily interdisciplinary team meetings and work with teams to enhance participants’ quality of life.
- Provide Case Management- Coordinate care with participants, families, and outside agencies to improve outcomes.
- Engage in Ongoing Education – Maintain certifications and stay current with best practices in an environment that promotes ongoing learning.
- Compensated On-call Rotation- Benefit from a paid on-call rotation once per month, ensuring time for both professional growth and personal life.
Skills, Knowledge & Expertise
- Diploma or degree from an accredited School of Nursing or University.
- Current Registered Nurse (RN) license to practice in the State of Michigan.
- Minimum of one year of experience working with the frail elderly or long-term care population.
- Previous experience in home care or long-term care is preferred.
- Strong clinical assessment and nursing skills with the ability to manage complex medical situations.
- Ability to work independently and as part of an interdisciplinary team (IDT).
- Knowledge of PACE-specific care, interventions, and regulatory standards is a plus.
- Excellent communication and interpersonal skills, with the ability to work effectively with participants, families, and the healthcare team.
