Job Summary/Overview:
To maintain the coordinated delivery of substance abuse prevention, promotion and education services to the patients we serve, community groups, schools and businesses; and to do other work as required.

Responsibilities: Include but are not limited to:
- Evaluate needs and design and implement substance abuse prevention forums and programs. Schedule SUD/MAT service appointments.
- Perform appointment reminder calls and perform follow up calls for missed appointments.
- Complete client paperwork including intakes and screenings. Maintain client level reporting system and perform data entry, as needed.
- Complete required external and internal program reports within timely manner.
- Maintain appropriate documentation in patient’s account to ensure patient charts are in compliance.
- Assist in quality assurance activities and conduct chart audits for MAT/SUD program.
- Coordinate and follow up on patient services, i.e, referrals for dental, mental health, etc.
- Prepare current and updated SUD/MAT educational and informational resources for distribution to patients.
- Conduct outreach and link patients to care.
- Attend all program related meetings and trainings.
- Travel to other clinic locations to meet with patients and trainings.
- Coordinate SUD/MAT services with Population Health, Maternal Health, and Behavioral Health department.
- Plan, coordinate, facilitate and present substance abuse prevention programs.
- Develop and maintain a cooperative working relationship with and between a wide variety of schools, community, media and professional representatives.
- Prepare and present oral and written materials convincingly, clearly and logically.
- Evaluate curricula and other educational materials related to substance abuse prevention.
- Refer the medication-assisted treatment (MAT) when needed.
- Able to identify, understand, and diagnose following the ASAM criteria.
Minimal Qualifications/Experience/Skills
- Bachelor’s degree in addiction studies, social work, or related field.
- Certified Alcohol and Drug Counselor (CADC) or equivalent
- 2 years preferred experience in SUD treatment center, recover home, or recovery environment.
- Working knowledge of the effects of drug and alcohol abuse and substance abuse treatment programs and prevention methods and techniques.
- Willingness to buy into and work under the guidance of our mission statement.
