- Complete initial in-home client evaluations and develop person-centered plans of care.
- Review and update plans of care at least every 90 days, or sooner as client needs change.
- Perform supervisory visits to clients’ homes to assess caregiver performance and ensure care plan compliance.
- Provide training, in-service education, and competency verification for in-home aides.
- Document assessments, supervisory visits, and care plan changes accurately and timely.
- Communicate with clients, families, and agency staff to coordinate and adjust care as needed.
- Ensure compliance with all applicable regulations and standards of practice.
- Participate in quality assurance activities and assist with audits or surveys.
- Active North Carolina RN license in good standing (or compact-state license valid in NC).
- Minimum of 1–2 years of nursing experience (home care or geriatric care preferred)
- Strong assessment and documentation skills
- Excellent communication and interpersonal skills
- Ability to supervise and support non-clinical staff
- Detail-oriented with strong organizational and time management skills
- Valid driver’s license and reliable transportation for home visits
- Familiarity with state home care regulations and best practices
