- Reviews the medical record for accuracy, compliance and completeness according to LCOH policies and regulations.
- Monitors Inpatient/Residential/Partial Hospitalization Program physician delinquencies for H&P’s, Inpatient Psychiatric Assessments, Discharge Summaries and Admission/Discharge orders.
- Monitors outpatient physician/clinician delinquencies by identifying any notes not entered in the EMR and encounters not closed.
- Scans all paper documents to correct patients’ medical record in EPIC including records received from external providers.
- Responds to requests for release of information to persons and agencies, according to regulations, in a timely manner.
- Logs all requests for release of information in the EMR.
- Creates invoices for billable medical record copies and manages payments
- Retrieves and files patient records for physicians and other medical personnel according to department procedures.
- Corrects errors made in EPIC according to department procedures.
- Enters all patient documentation into patients’ records after EPIC downtime according to department procedures.
- Performs other duties as assigned.
- High school diploma required.
- RHIT preferred
- At least 2 years’ experience working with patient medical records.
- Experience with Electronic Medical Record (EMR) preferred.
- Office software experience (Word, Excel, and Outlook) preferred.
- Experience with office machines (copiers and fax) preferred.
- Ability to multi-task.
- Excellent communication skills and strong customer service skills are required.
