About This Position
The Community Health Liaison will support the City of Newark Police Department Crisis Assistance Response and Engagement (CARES) program and will serve as a liaison among providers, hospitals, peer counselors, and people seeking treatment for and addressing their mental health needs.
PLEASE NOTE: This position will be working out of the Newark Police Department, 220 South Main Street, Newark, DE 19711
Principal duties and responsibilities:
- Conduct interviews with all new participants to establish baseline data at intake.
- Work with the clinical professionals to identify/explain appropriate level of care.
- Present treatment options, inclusive of co-occurring disorders if necessary.
- Conduct weekly wellness checks to confirm compliance with treatment.
- Work closely with the police officer and health care professional (Social Worker, MSW) within the City of Newark Police Department to track that the level of care remains consistent and fitting for everyone evaluated by the team.
- Maintain open communication with the individual, their families and friends to ensure their safety.
- The Community Health Liaison will create a folder for each participant to include intake interview data, and the evaluation of the option for qualifying participants to enter transitional or recovery housing as part of a comprehensive response strategy (if applicable).
- Provide direct services by creating a single coordination point for individuals seeking treatment.
- Coordinate community outreach.
- Coordinate training for Newark officers regarding available referral mechanisms and developing strategic partnerships to increase program efficacy.
- Establish and maintain partnerships with treatment providers, health services, and support groups to enable efficient and comprehensive service delivery for program participants.
- Will convene regular stakeholder discussions surrounding project implementation.
- Complete basic intake notes to include contact information and summary needs assessment.
- Make sure medical authorization releases are signed for each level of care.
- Establish point of contact with participants’ treatment provider/TASC/Probation and provide progress updates as needed.
- Document progress updates.
- Coordinate case management support with appropriate Newark Police officer, as needed.
- Accompany participants to Court or coordinating one-on-one meetings if necessary.
Education and experience requirements:
- Minimum of a bachelor’s degree in a related field. BSW preferred.
- 2-3 years’ experience working with treatment providers; law enforcement and social service providers.
