Responsibilities:
- Conduct medication reviews and assessments to ensure optimal therapeutic outcomes.
- Provide clinical consultation to healthcare teams and nursing staff on medication-related issues.

- Monitor patient medication regimens for safety and efficacy, adjusting as necessary.
- Educate patients and staff on medication management and best practices.
- Stay current on pharmacy regulations and industry trends to ensure compliance.

- Participate in quality assurance initiatives and pharmacy audits.
Requirements:
- PharmD degree from an accredited pharmacy school.
- Active pharmacist license in California.
- Minimum of 2 years of experience in a clinical pharmacy setting.
- Strong knowledge of pharmacotherapy and medication management.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced environment.
