The University of Maryland Medical Center Midtown Campus, part of the renowned University of Maryland Medical System, is seeking an experienced and highly skilled Nurse Practitioner (CRNP) to serve as Manager Clinical Program – Heart Transplant.


This is a unique leadership opportunity to join a high-performing transplant program within an academic health system known for clinical excellence, innovation, and multidisciplinary collaboration. The role combines advanced clinical practice with program leadership to support the delivery of exceptional care for heart transplant patients across the continuum.


Position Overview

The Manager Clinical Program – CRNP for Heart Transplant is responsible for providing advanced clinical care while supporting the operational, clinical, and quality performance of the heart transplant program. This role works closely with physicians, surgeons, and multidisciplinary transplant teams to ensure safe, high-quality, and patient-centered care.


The CRNP will function as both a clinical expert and program leader, contributing to the management of complex transplant patients and helping drive program growth, quality outcomes, and regulatory compliance.


Key Responsibilities

Clinical Practice

  • Provide advanced practice care to heart transplant patients across inpatient and outpatient settings
  • Conduct comprehensive assessments, physical examinations, and diagnostic evaluations
  • Order, interpret, and manage laboratory and diagnostic testing
  • Develop and implement individualized treatment plans in collaboration with transplant physicians
  • Manage immunosuppressive therapy and post-transplant complications
  • Support continuity of care throughout the transplant lifecycle

Program Leadership & Coordination

  • Collaborate with transplant surgeons, cardiologists, nurses, and allied health professionals
  • Participate in multidisciplinary rounds, case reviews, and care planning
  • Support adherence to UNOS, CMS, and institutional transplant regulations
  • Assist with program development, quality improvement, and performance initiatives
  • Contribute to transplant education, protocol development, and best practices

Quality & Outcomes

  • Monitor patient outcomes and support continuous quality improvement initiatives
  • Assist in tracking transplant metrics, clinical benchmarks, and regulatory compliance
  • Participate in data review, reporting, and program evaluation activities
  • Promote evidence-based practice and clinical excellence

Work Experience

Qualifications

Required

  • Certified Nurse Practitioner (CRNP) licensure in Maryland or eligibility to obtain
  • Acute Care Nurse Practitioner certification required
  • Strong clinical background in adult gerontology, cardiology, critical care, or transplant medicine preferred
  • DEA registration (or eligibility)
  • BLS and ACLS certification

Preferred

  • Experience in heart transplant or advanced heart failure care
  • Prior leadership or program coordination experience in an advanced practice role
  • Strong understanding of transplant regulations (UNOS/CMS)
  • Excellent communication, leadership, and collaboration skills
  • Ability to manage complex, high-acuity patient populations
