Responsibilities
- Provide emergency nursing care and rapid assessment in a home hospice setting when acute changes occur.
- Monitor patient condition, vital signs, and respond to clinical changes with appropriate interventions.
- Administer medications and treatments per physician orders; manage IV therapy as directed.
- Collaborate with the interdisciplinary hospice team and on-call physicians; document care accurately in the charting system.
- Educate patients and family members on symptom management, safety, and care planning; support comfort and palliative goals.
- Ensure infection control, safety practices, and adherence to HLFS standards; escalate concerns as needed.
Requirements
- Active California RN license
- Certifications: Basic Life Support (BLS); ACLS; Hospital Fire and Life Safety (HLFS) required; BLS may be HS-BLS or RQI BLS per policy
- Education: RN degree from an accredited nursing program (ADN or BSN)
- Experience: ER or acute care experience preferred
- Ability to work in a home health/hospice setting and participate in a travel assignment with 3x12-hour day shifts
