You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.Location: CentralResponsibilitiesProvide nursing care according to the RN‑developed Plan of Care.Monitor and document changes in patient condition; report to RN/physician as needed.Assist with activities of daily living to promote patient independence.Educate patients and caregivers within LPN/LVN scope of practice.Maintain accurate EMR documentation.Participate in case conferences and coordinate care with the home health team.Follow infection‑control, HIPAA, and company policies.RequirementsCurrent LPN/LVN license in the state of practice.Graduate of an accredited practical/vocational nursing program.Minimum 1 year acute care experience preferred.Current CPR/BLS certification.Valid driver’s license, reliable insured vehicle.Strong communication, clinical judgment, and ability to work independently.
