POSITION SUMMARY
The Housing Navigators will engage and assist homeless participants identified by stake holders who are in hotels, shelters, and other locations into housing. This will include supporting individuals as they move into a short-term housing solution first and maintaining contact and engagement as required to assist the participant with transitioning from short-term housing to either permanent supportive housing or subsidized housing.
Pay Range: $23.03 per hour
POSITION SUMMARY
The Housing Navigators will engage and assist homeless participants identified by stake holders who are in hotels, shelters, and other locations into housing. This will include supporting individuals as they move into a short-term housing solution first and maintaining contact and engagement as required to assist the participant with transitioning from short-term housing to either permanent supportive housing or subsidized housing.
MINIMUM QUALIFICATIONS
Bachelor’s degree plus two (2) years’ experience in social services (case management experienced preferred). Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred. An automobile, valid Driver’s License and vehicle insurance coverage is required. Occasional late or weekend hours may be required.

RESPONSIBILITIES
•Receive referrals for families and individuals experiencing homelessness through sake holders.
•Orient households to the housing process and to the rights and responsibility of tenancy.
•Assist the household in scheduling and arranging travel appointments for unit viewings and lease signings.
•Review and explain lease and other housing documents with the household.
•Collaborate with housing providers to learn what documents are needed for the program and help household obtain documents, as conditions allow. This includes assisting with obtaining identification documents and completing intake paperwork.
•Enter all case notes using the DAP format.
•Collaborates with housing programs to assist former participants if any issues should arise with housing placement.
•Ensures effective communication with participants, providers, landlords, and
Stake holders.
•Enters accurate and timely data for assigned households into HMIS.
