JOB SUMMARY:
Provides personal care services to clients in the GAPS, Level Up, and Safe Haven programs.
JOB DUTIES/ESSENTIAL FUNCTIONS:
• Provide training to clients following goals developed in their service plan. Training may include money management, personal hygiene skills, grocery shopping, housekeeping, preparation of meals, laundry skills, and community integration.
• Document daily activities and training provided to clients on shift logs and notes. Documentation should follow agency guidelines and state licensing/certification requirements.
• Plan and conduct structured activities for participants, in conjunction with the Program Director and/or Case Manager.
• Coordinate with the client’s Case Manager regarding resident progress, appropriate service plan updates, and training activities, including money management, personal hygiene skills, grocery shopping, housekeeping, preparation of meals, laundry skills, and community integration.
• Ensure that clients attend medical appointments as scheduled, in conjunction with the client’s Case Manager.
• In the event of illness or injury, take appropriate steps to ensure medical attention is received, if necessary. Report all illnesses or injuries to the Program Director or supervisor on call.
• Assist with self-administration of medications as prescribed and follow doctor’s orders while following agency guidelines and state licensing/certification requirements.
• Procure refills of medications in a timely manner. If the Residential Monitor cannot obtain the refill, the Case Manager should be included for assistance.
• Complete documentation regarding assistance with self-administration of medications or other doctor’s orders.
• Transport clients to appointments or social events as needed.
• Assist with securing the building by monitoring entrances, completing routine walkthroughs, checking cameras as needed, and reporting any safety concerns or unusual activity to the appropriate supervisor.
• Assist with maintaining a clean, safe, and orderly environment by completing light housekeeping duties in common areas as needed.
• Report all relevant information regarding the residents, building, and/or incidents to the next shift.
• Participate in safety drills.
• Complete critical incident reports as needed. Follow policy for reporting.
• Document complaints made by client and/or family. Report to supervisor.
• Maintain client confidentiality per HIPAA standards.
• Identify and handle crisis situations and contact Program or Division Director for needed support and direction.
• Identify and report maintenance problems to the Property Manager.
• Develop and maintain positive relationships with agency staff members, outside agencies, and the local community.
• Attend staff trainings and meetings as required by VOANLA.
• Complete all trainings required by program.
• Work on special projects as assigned.
REQUIRED EDUCATION AND EXPERIENCE:
• High school diploma or GED.
• Prior work experience in a program that serves persons with disabilities preferred.
• CMA certification preferred.
• Current CPR/First Aid certification required or must obtain upon hire.
• Current valid driver’s license with acceptable driving record.
• Automobile liability insurance that meets minimum requirements.
