With a staff of 950+ and an annual budget of $140 million, Project Renewal is one of the larger social service nonprofits in New York City.   Title: Entitlements Coordinator Program & Location: Barbara Kleiman Men's Shelter  - Brooklyn Salary Range: $ 43,000-$45,000   Program Overview:   The Barbara Kleiman Men’s Shelter is a 200-bed, 24/7 emergency shelter for 200 men who are experiencing homelessness.  The program will serve individuals diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, mental health care and primary medical care, recreational activities, occupational therapy, employment services and job readiness services, and housing placement assistance.   Essential Duties & Responsibilities:   The essential duties of the Entitlements Coordinator include but are not limited to the following activities:   Assists clients with access to available public benefits including but not limited to Public Assistance, SNAP, public health insurance, and Federal disability benefits Makes eligibility determinations, processes applications, and completes all related follow-up Develops resources and contacts to facilitate clients’ access to benefits in a timely manner Complete case notes for all interactions with clients and for collateral contacts with outside agencies Escort clients to Fair Hearings and other HRA appointments, as necessary Works in conjunction with the Case Managers, other clinical department staff members to schedule/accompany clients to appointments for entitlements and healthcare Coordinates reminders and transportation arrangements to ensure that appointments are kept Provides coaching to clients about how to maximize and keep their benefits active Provides clients and staff with updated information and direction in acquiring and maintaining entitlements with city, state, and federal entitlement programs Assist clients in completing medical registration packets and creating initial charts in eCW Assist in scheduling clients for initial and follow-up appointments with onsite psychiatric provider Performs other duties as assigned by the Program Director and or Director of Social Services Physical Activities   While performing the duties of this position, the Entitlements Coordinator must stand to talk to clients and staff, sit for extended periods of time at the computer, and reach or squat to get records from the files.  The Entitlements Coordinator must be able to use the New York City Transit system to escort clients to scheduled appointments, if needed.   Qualifications:  High School Diploma or GED required and A minimum of one (2) years of experience working with in a non-profit or government organization providing social services to a target population which includes but is not limited to the homeless, mentally ill, substance, use, HIV/AIDS, criminal justice, and LGBTQIA+ Preferred Skills:   Team Building:                  Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible  Organizational:                 An ability to work independently and to adjust one’s schedule to accommodate the needs of the clients Communication:               Excellent oral, writing, and listening skills Computer:                           Knowledge of Case Management software (CARES) Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.  Familiarity with eClinicalWorks or other medical software a plus Language:                            Bilingual or multilingual a plus   Project Renewal is an equal opportunity employer.  Its long-standing policy has been to embrace the equality of opportunity for all employees and applicants without regard to actual or perceived race, color, national origin, alienage or citizenship status, religion or creed, gender (see separately issued Gender Discrimination policy for more detail), physical or mental disability, age (18 and over), military status, arrest record, marital status, domestic partnership status, genetic information or predisposing genetic characteristic, sexual orientation, status as a victim or witness of domestic violence, sex offenses or stalking, unemployment status or any other basis protected by applicable federal, state and local laws.
