Job Description
Title: Nurse Manager, Residences, PSTP Program– Healthcare Department
Program: Nursing. PSTP Program
Salary: $80,000 - $85,000 at 35 hours/week
Position Overview:
Reporting to the Deputy Director of Nursing and to the Director of Nursing Operations (DNO). The Nurse Manager for Residences, PSPTP Program will provide direct patient care while also supporting the daily nursing operational needs for nursing residences and PSTP Program in the Project Renewal’s PSPT Program and Housing HASA/OASAS/OMH sites.
Essential Duties & Responsibilities:
Residences Clinical
Ensures all residential embedded nurses are keeping accurate patient self-administration records
Ensures all residential embedded nurses are keeping consents, accurate patient self-administration records, locked controlled medication boxes.
Ensure all nurses are using EMR for appropriate documentation of patient teaching, medication, and injection administration.
Responsible for the administration of medications including vaccinations; aware of the location of all medication and supplies for emergency usage; keeps medication stocks current.
Performs procedures including injections, fingerstick glucose testing, and pulse oximetry as ordered by the clinician.
Responsible for patient education, per the Health Center policies and procedures.
Participates in emergency care as necessary
Attends staff meetings and high alert case conferences
Documents patient interactions in the Electronic Health Record
Responsible for safe practices of medication management safe keeping of temperature logs and emergency logs handling excursions, medication errors or incidents.
PSTP Clinical
The RN manager will coordinate with the client’s psychiatry provider and case management team to highlight and target priority patients and assist by providing the team with clinical feedback and assessments.
PSTP home visits will be based on internal referrals from Case Management, Housing Counselors, Psychiatry, and Program Directors.
Referrals for clients will be communicated to the RN Manager during the weekly case conference meetings.
The RN Manager will have a panel open to document visit notes, coordinate care with other clinicians and or social services team. (Note: social services will not have ECW access)
The RN manager will work closely with PSTP leadership for data reporting, grant funded audits providing clinical information, assuring policies and procedures are being followed, and coordination with house aide and case management team.
The RN manager will delegate to staff LPN to collect medication compliance intake forms.
Other duties as assigned.
Nursing Operations
Maintain, document, and report out inventory of medical equipment, supplies, and medications under guidance of Dpty DNO and DNO
Oversee processes to ensure audit compliance for medications and medical equipment.
Oversees ordering procedures and stocking of medical equipment, supplies, and medications and distribution of medical equipment, supplies, and medications from headquarters to clinical sites.
In conjunction with Dpty DNO and DNO, maintains documentation in compliance with agency policies and procedures and maintains updated knowledge of policies and procedures affecting the organization and assists in interpreting or implementing them as necessary.
Works closely with the Dpty DNO and DNO to develop and improve systems for healthcare within Project Renewal.
Keeps Healthcare Department leadership informed of all issues and developments and provides a thorough, comprehensive, and concise report of the activities, as needed.
Qualifications:
New York State licensed Registered Nurse (RN)
Experience
Prior experience working with homeless individuals preferred.
3-5 years of clinical experience in outpatient primary care
Prior managerial experience a plus
Skills:
Communication: Must have excellent oral and listening skills.
Interpersonal skills: Must have the ability to interface with clients as well as all levels of staff.
Computer skills: Must have knowledge Microsoft Office, especially Word and Excel. Familiarity with eClinicalWorks preferred
