Phoenix House is seeking a Licensed Practical Nurse to support clients' health, safety, and recovery in a behavioral health and substance use treatment setting. The Licensed Practical Nurse will perform the following tasks: 

  • Provide direct nursing care to clients under the supervision and direction of clinical leadership. 
  • Maintain patient confidentiality in accordance with Phoenix House policies and procedures. 
  • Oversee unlicensed assistive personnel (e.g., CNAs, Medical assistants, and Medical Technicians). 
  • Perform routine nursing procedures such as EKGs, wound care, and specimen collection.  Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.  
  • Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership. 
  • Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership. 
  • Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies. 
  • Other duties as assigned.

REQUIRED EDUCATION

  • High School Diploma or GED
  • Graduate of an accredited nursing program

LICENSES/CERTIFICATIONS

  • Current NYS Licensed Practical Nurse
  • First Aid Training 
  • CPR Training
  • Narcan Training
  • Infection Control Certification
  • Child Abuse Certification

REQUIRED EXPERIENCE

  • At least three years of related experience
    • Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs.

WORK ENVIRONMENT

  • This job operates in an indoor medical clinical environment. This role also routinely comes into contact with patients who may have contagious illnesses.
