Hospice: Social Worker (LSW/LGSW/LISW/LICSW) - North Central

The Social Worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement which include providing care based on the Social Work Practice Act.

Position: Social Worker for Hospice Wage: $56,000-$88,000 annually, depending on experience -  (wage dependent on license: LSW, LGSW, LISW, LICSW) Service Area: North Central (see area of coverage in the post). Plus, mileage reimbursement of .72 1/2 cents per mile. Schedule: Full-Time / Monday – Friday, 8:00am-5:00pm, 1.0 FTE
