Job Description:

Schedule: Variable day shift times based on need.

Location: 601 E Rollins Street, Orlando, FL 32803

Educate and counsel patients, families, and healthcare team members on safe and effective medication use. 

Complete Discharge Medication Reconciliations (DMRs)
• Ensure medication history and admission medication reconciliation are entered appropriately
• Resolve incorrect doses, outdated meds, and inappropriate therapy based on evidence-based guidelines by communicating with patients, caregivers, and providers
• Reconcile inpatient orders with provider recommendations and patient’s current health status to ensure discharge orders are appropriate
• Work with inpatient case management and outpatient pharmacy services to ensure patient has access to medications upon discharge
• Provide medication counseling including recommended drug monitoring and disease-specific goals when appropriate
• Triage and resolve issues that arise post-discharge when appropriate
Participate in Quality Improvement Initiatives
• Readmission reduction tracking
• Medication discrepancy resolution and interventions
• Cost-avoidance programs
• Length of Stay initiatives
• Pharmacy student and resident precepting

• 1+ hospital experience strongly preferred [Preferred]

Pay Range:

$50.49 - $93.91
