Job Description
The Supported Housing Case Manager works with clients residing in the Supported Housing buildings to provide behavioral health treatment in the client's natural setting. The case manager is responsible for completing assessments, developing an individual service plan, and monitoring progress. The case manager provides case management both on site and within the community. Case Manager assists clients with fostering independence and maintaining safety and stability while maintaining housing and benefits and improving self-determination through illness management. The case manager coordinates care with the client, family/natural supports, and outside providers.
Sunday - Thursday 8:30am-5:00pm
