URM Aftercare Case Manager

The URM Aftercare Case Manager is primarily responsible for providing culturally sensitive social and case management services to youth who have discharged from the URM program to include but not limited to creating service plans, assisting with the youth’s safety, maintain accurate and up-to-date documentation of all provided services in accordance with all applicable regulatory and accreditation standards.
