South Loop Physicians Group is an offsite, university-supported primary care clinic affiliated with the University of Illinois Chicago Department of Medicine. The Clinic Nurse provides professional nursing care in an ambulatory setting, supporting adult, pediatric, and specialty primary care services. This position delivers direct patient care, administers medications and vaccines, coordinates care with interdisciplinary teams, supports teaching and quality improvement initiatives, and contributes to research and community-focused clinical programs serving diverse and vulnerable populations. Clinical Services Provided: Michael Reese CARE Program Internal medicine primary care Pediatric primary care Lactation support Geriatric primary care Urogynecology Two-Generation Care for Mothers and Infants Adults and children with disabilities primary care Duties Responsibilities: Completes required orientation, annual competencies, and unit-specific training. Conducts ongoing patient assessments and develops, implements, and updates individualized plans of care in collaboration with patients, families, and providers. Administers medications and vaccines in accordance with provider orders and established clinical protocols. Verifies, implements, and documents physician, physician assistant, and advanced practice nurse orders in the electronic medical record. Educates patients and families regarding treatment plans, medications, preventive care, and follow up needs. Monitors patient conditions, recognizes changes in status, establishes priorities, and responds appropriately to clinical needs. Coordinates medication prior authorizations and supports continuity of care. Maintains vaccine inventory and manages responsibilities related to the ICARE program. Collaborates with interdisciplinary healthcare teams to support high-quality patient care, teaching, and quality improvement initiatives. Assists with patient scheduling and clinic workflow as needed. Perform other related duties and participate in special projects as assigned.
