You will have responsibilities including but not limited to:

  • Facilitates waiting lists for patients and families as a means of support for them while awaiting treatment services.
  • Coordinates and completes the admission and discharge process to maintain an accurate daily census
  • Coordinates and completes the admission of patients with approval by their authorized representatives and/or funding agencies.
  • Performs assessments, screening, and crisis intervention with patients as required.
  • Evaluates test results, psych evaluations and other reports/interviews to determine if candidate is appropriate for treatment at Phoenix House.
  • Maintains accurate and timely documentation in accordance with the expectations and standards of Phoenix House policy and procedures and state licensing requirements. Maintains patient records utilizing the Patient Information System, the Patient Management System and BHIPS.
  • Conducts routine follow up on all patients once discharged.
  • Participates in clinical reviews, staff meetings, trainings, and other meetings in accordance with program needs.

 

Job Details:

  •  Ronkonkoma Residential - Ronkonkoma, NY
  •  Full Time-exempt
  •  Salary range $58,000 - $68,000.

REQUIRED EDUCATION

  • Masters Degree in Social Work, Mental Health Counseling or related field

 

LICENSE/CERTIFICATIONS

  • NYS LMSW, LMHC in good standing required
  • CASAC preferred

 

REQUIRED EXPERIENCE

  • Minimum three years working in substance use treatment admissions
  • Familiarity with regulations governing substance use treatment services

  • This job operates in an indoor office environment. This role routinely uses standard office equipment such as computers, phones and photocopiers.
