
Bradford Aftercare Specialist
Job Description
Summary of Position:
The Aftercare Specialist is a detail-oriented team player who will be responsible for the coordination of post treatment care and appointments. Including housing, medical, and personal care. This person will participate in treatment team meetings and discuss patients’ emotional/behavioral issues/concern, and related programming needs.
Duties and Responsibilities
• Assists Primary Therapists in Treatment Plan development and monitors updates for a limited number of clients
• Ensures preliminary assessment requirements are completed for assigned co-primary caseload (PPLOS, ASI-MY, ASI Addendum, etc.)
• Confers and collaborates with Primary Therapist to ensure the clinical best treatment interests of all patients are met
• Attends weekly Clinical Team Meetings when available
• Facilitates program-specific small groups and co-facilitates clinical process groups
• Develops and facilitates program specific psycho-educational group module
• Coordinates and documents as necessary, daily activities (telephone calls, read/edit assigned task work, etc.
• Coordinates with Primary Therapist to meet essential case management components such as continued care plan appointments, aftercare service-provider appointment, etc.
• Covers caseload with Lead Recovery Aide and/or Clinical Manager in Primary Therapist absence
Qualifications Required to Perform Essential Job Functions:
Highschool diploma/GED preferred; or six months related experience and/or training; or equivalent combination of education and experience. Associates degree (A.A.) in related field and knowledge of chemical dependency and mental health preferred.
