GENERAL STATEMENT OF RESPONSIBILITIES:

The Women & Children’s Service Coordinator is responsible for coordinating, and scheduling all admissions for the program. They are the primary contact for all referrals to the program and will be the primary point of contact for other providers, referral sources and potential clients. The Service Coordinator will promote engagement and retention in treatment and facilitate an effective discharge plan whereby the individual successfully connects to the next level of care. The Service Coordinator will work with a multidisciplinary treatment team to help individuals engaged in the Women & Children Program to achieve their recovery and parenting goals. This position required flexibility with location, schedule and may include evening or weekend hours.

EXAMPLES OF RESPONSIBILITIES:

  1. Complete pre-screening assessment with women referred to the program. Present referrals to the program team and collaborate with Department of Children and Families and Department of Mental Health and Addiction Services to ensure eligibility.

 

  1. Schedule incoming individuals for admission, collaborating with Medication- Assisted Treatment Programs to ensure medications are available to client at intake, any other providers involved in client’s care and connecting clients with transportation to the program.

 

  1. Work with the treatment team to ensure all case management needs identified for client are obtained for the client and her child(ren). Responsible for obtaining resources, connection to other programs and other needs are met.

 

  1. Facilitate group sessions utilizing evidence based and research driven curriculum and interventions on topics related to substance use disorders, obtaining basic needs, budgeting, job searching, obtaining education goals, discharge planning and parenting.

 

  1. Met with each individual engaged in the program during intake and throughout treatment to develop an appropriate discharge plan. Assist individuals to follow through with referrals to other programs for themselves and their child(ren), including but not limited to medical appointments, behavioral health appointments, housing, job searching, etc.

 

  1. Participate in and advocate for our individuals in family meetings facilitated by The Department of Children and Families and other case conferences that involve clients’ other resources and programs.

 

  1. Develop effective networking relationships with outside contractors, referral sources and community agencies. Engage in ongoing marketing of programs and represent the agency at community events and meetings as assigned by Management.

 

  1. Provide written documentation on client’s status to clients, referral sources, Department of Corrections, Court Support Services, Department of Children & Families, Employers, Housing programs, etc.

 

  1. Assist with the day-to-day residential component of the program. May include supervising smoke breaks, supervised drug screening, supervision of Self- Administration of Medication, transportation and supporting clients in completing their activities of daily living (ADLs) for themselves and their child.

 

  1. Participate in supervision, team meetings, rounds, agency committees and work groups as assigned by management. May be required to participate in trainings (both on site and off site) as determined by the Director of Residential Services and/or Vice President of Adult Services.

 

  1. Provide coverage for coworkers during time off and to help ensure required residential staffing levels are met at all times.

 

  1. Complete other duties as assigned.

 

EDUCATION / EXPERIENCE:

A master’s degree in Social Work along with a Connecticut State license from the Department of Public Health (LMSW or LCSW). A Connecticut license as a Professional Counselor or Marriage and Family Therapist (LPC-A, LMFT-A) with experience in working with clients diagnosed with substance use disorders will be considered. L.A.D.C is not required but welcomed in addition to primary licensure.

Training preferred on the following topics; Treatment Criteria for Addictive, Substance-Related, and Co-Occurring Conditions (ASAM), Co-occurring disorders, including prevalence, signs/symptoms, assessment, treatment and the impact of one’s own attitudes/beliefs on the delivery of services and the use of naloxone in response to an opioid overdose.

LICENSURE/CERTIFICATION/REGISTRATION:

Valid driver’s license with clean driving record is required and must be CPR and First Aid Certified.

Must demonstrate computer competency in Windows and Outlook with ability to quickly learn and accurately maintain agency-specific electronic files. Must display willingness and ability to adapt to new situations and challenges. Must consistently maintain a pleasant, professional attitude. Must demonstrate an ability to exercise exceptional judgment and deliver meet high standards of customer service. Must exhibit initiative and accept accountability for tasks. Must demonstrate and ability and willingness to work a flexible schedule, including coverage for co-workers during vacations, illness. Including day, evening and weekend hours as needed. Must be able to work mandated shifts due to staff shortage, weather, or emergent events.
