Support Whole-Person Care as a Nurse Care Manager in Our Behavioral Health Healthcare Home

Key Responsibilities
- Coordinate care for up to 200 clients, completing health screenings and identifying physical health risks
- Support wellness planning by setting physical health goals and promoting healthy lifestyle changes
- Educate clients on chronic condition management, appropriate healthcare usage, and health prevention
- Complete screenings (e.g., metabolic syndrome), medication reconciliations, and health consultations
- Collaborate with PCPs, specialists, and community partners to ensure holistic, person-centered care
- Participate in interdisciplinary team meetings and provide nursing support across clinic locations as needed
Requirements, Skills, Knowledge and Expertise
• Practical Nursing or Associate of Nursing degree required
• Bachelor’s degree in nursing preferred
QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
• Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
• A valid driver's license and agency established minimum automobile coverage required
QUALIFICATION(S): LICENSURE/CERTIFICATION
• Licensed Practical Nurse (LPN) required
• Registered Nurse (RN) preferred
