University Home Care Inc is seeking a compassionate and dedicated Registered Nurse Home Health to join our team in Livonia, MI. This role involves providing high-quality nursing care to patients in their homes, promoting health, managing chronic conditions, and supporting recovery. Responsibilities
- Provide skilled nursing care to patients in their homes according to physician orders and care plans.
- Assess patients' health status and needs, develop individualized care plans, and document progress.
- Administer medications, treatments, and therapies as prescribed.
- Educate patients and their families on disease management, prevention, and wellness.
- Collaborate with interdisciplinary healthcare team members to coordinate care.
- Maintain accurate and timely clinical documentation and comply with all regulatory requirements.
- Monitor patient safety and report any changes or concerns promptly.
Requirements
- Current valid Registered Nurse (RN) license in the state of Michigan.
- Minimum of 1 year of nursing experience, preferably in home health or related field.
- Knowledge of home health care regulations and nursing best practices.
- Strong assessment, clinical, and organizational skills.
- Excellent interpersonal and communication skills.
- Reliable transportation to travel to patient homes in Livonia, MI area.
